Project: (drop-down menu) Select the project for which you are evaluating the linguist.
Date: (calendar) Choose the date of the evaluation.
Locale: (drop-down menu) Choose the language-COUNTRY code which corresponds to the performed task. This is important because some linguists handle more than one locale (e.g., Russian for Moldova or for Belarusian).
Instrument type:
(drop-down menu) Choose one option from the menu:
Article
(e.g., short Web articles),
Assessment
(any kind of assessment),
Questionnaire
(e.g., background questionnaire in PISA/PIAAC etc.),
Reports
(e.g. OECD report, score reports),
Survey
(e.g., Satisfaction survey, HR surveys, opinion polls, etc.),
Survey/Test Responses
(specific tasks on survey or test responses),
Other
(can be specified in the Comments, e.g., website review, audio review).
Task: (drop-down menu) This drop-down menu includes all tasks we generally handle, however, if a specific task is not included in the list, choose Other and specify in the Comments field what task it was.
Feedback sent: (drop-down menu) Choose Yes, live if the feedback was given in person (for instance, during a seminar or through a skype call). Choose Yes, mail if an initial or final feedback was sent by mail. If any other means is used, please choose Yes, other and specify in the Comments field.
Timely Delivery: (drop-down menu) Choose Yes if the subcontractor delivered on time, No if there was a delay in the delivery (unless an extension was required and then kept or there were good reasons for the delay). N/A can be chosen if there was no specific deadline. Again, you can use the Comments field to specify your choice.
Compliance with instructions: (drop-down menu) If there were specific instructions, were these followed? Choose one of the options in this menu and specify in the Comments field if needed.
Responsiveness: (drop-down menu) Did the vendor reply to queries, mails, etc. in a timely manner? Choose one of the options in the list and specify in the Comments field if needed.
Added by: If it is not automatically filled in, type your name.
Overall evaluation: (drop-down menu) Choose among Excellent, Good, Average, and Poor to evaluate the performance of the vendor for this task. In your choice, please also take into account if the vendor is a newbie or an old-hand.
Comments: (free-text field) Note: this field is only visible when one clicks on a line to see the specific evaluation. You can add any details or specific comments here or explain your choices in the previous fields if needed. Comments do not need to be too wordy, but some extra information can be useful to colleagues.
Examples: Very accurate. Tends to write long comments. Instructions are internalized. Since she works full-time, availability and responsiveness can at times be on the down side.

Seems to be online all the time, responses are always very quick. VERY meticulous with the tendency to overdo things, so needs precise instructions and/or feedback, otherwise both him and the reviewer may use too much time in what is not in our job description.
